Overview

Patients complete mobile-friendly forms with conditional logic, auto-save, file uploads and pain diagrams before the visit, and sign consent and financial documents electronically, including multi-signer forms. Form data is mapped onto the practice's own PDFs, and staff receive an email alert with the completed forms attached as PDFs. MedForward builds and maintains the forms, PDF mappings and routing, runs each account on a dedicated server with a signed BAA, and offers a REST API for retrieving submissions.
